What is the Vision Benefit?
The vision benefit is $150 every 24 months. This covers $30 for an eye exam, plus $60 for frames and $60 for lenses or $120 for contact lenses. At a participating provider, there’s no co-pay and your contacts or glasses are free, if you choose from the wide selection of frames that the benefit covers. If you choose a frame that the plan doesn’t cover, you must pay the difference in cost.
If you go out-of-network for eye care, you’re also covered for up to $150 of care every 24 months, at the same rates listed above. You pay the bill and submit a claim to the Health Fund for reimbursement.
