How To Appeal a Denied Claim
If you have received health services and your medical or hospital claim is denied, you have the right to appeal. Here’s how:

CALL: Empire BlueCross/BlueShield at (866) 316-3394 or

WRITE
to:
Empire BlueCross BlueShield
Attn: Medical Management
Appeals
Mail Drop R/6 O
P.O. Box 11825
Albany, NY 12211
You must appeal within 180 days from the date that your claim is denied or your right to appeal is waived.

If Empire upholds the denial, you can file a voluntary appeal with the Health Fund’s Appeals Committee by writing to:
32BJ Health Fund
Board of Trustees
Appeals Committee
101 Avenue of the Americas
New York, NY 10013
You can only request that the Appeals Committee review your claim after Empire has upheld the denial of your appeal. A voluntary appeal must be made within 180 days from the date Empire notifies you of its decision. If you do not appeal within this time frame, you give up your right to appeal.
