For the Direct POS in Metro NYC area

• Click on Find a Doctor
   
• Click on In Empire’s local area network (Only for Metro NY)
   
• To select your Health plan (check the back of your Empire Card to be sure of the network you are in)
   
• Select Type of Provider you are looking for
   
• Select Specialty, if needed
   
• You can find a doctor by putting in the provider’s name, if not enter your address/location
   
• Once completed, click Show Results.

For the PPO outside NYC Metro Area - For example: CT, Philadelphia, DC/Maryland

• Click on Find a Doctor
   
• Click on Across the Country
   
• Choose PPO/EPO as your health plan
